1. Application Form
You will need to download our application form and fill this in - Please note that any gaps in your employment history will need to expand using form 8.
2. Fitness to Practice Declaration
You need to read and confirm your Fitness to Practice, you will only need to use this form if you are applying for a Nursing, teaching or if your recruitment consultant has asked you to do this,
3. Declaration of Good Character
You need to read and confirm you're Good Character, you will only need to use this form if you are applying for a Nursing, teaching or if your recruitment consultant has asked you to do this
4. Confidentiality Agreement
You will need to read and sign our Confidentiality agreement. If you have any concerns using this. please talk to your consultant.
5. Health Declaration
It is helpful for us to know about any reasonable adjustments that we make for you to support you through the recruitment process and into employment. please read this form and any necessary comments.
6. Reference Request Form
We are required to collect up to three references on you. One must be a current employer/tutor. One should be a professional and the last one personal. Please use the form provided
7. CPD Log
you will only need to use this form if you are applying for a Nursing, teaching or if your recruitment consultant has asked you to do this
8. Gaps in Employment (if applicable)
We are required to ensure that we check your full working history for the past five year's some of the roles require 10-year checks. the recruitment consultant will advise you.
9. 48Hr Opt out an agreement
By law you are not allowed to work on average (taken over a 17 week period) more than 48 hours per week. This should be signed and returned should you wish to opt out of this law.
10. Criminal Conviction Declaration
Please provide details of all unspent criminal convictions. If you posses CRB clearance, please provide a clear photocopy.
11. PAYE Contract
This is your contract of employment with us. Please read carefully, sign the back page and return.
12. Temporary Workers Handbook
A useful handbook to update you on what you can expect from us and what we expect from you.
13. Weekly Timesheet
Please download, ask your line manager to complete, sign and return to us by email (email@example.com by 3pm every Monday to ensure it is included in the weekly payroll.
14. Multi Timesheet
A weekly timesheet for reporting multiple candidate jobs. Please ensure all timesheets are emailed by 3pm Monday following the week worked.
15. New Starter Checklist (HMRC)
Any employee commencing work without a P45 should complete and return this form to ensure they are issued with a valid tax code.
